Key Findings

Semaglutide has the worst severity profile — 30.1% of side effect reports rated severe, vs 26.8% for tirzepatide and 21.9% for retatrutide. This is driven by its intense GI burden.

Retatrutide has unique non-GI side effects largely absent from the other drugs: heart rate increase (7.2%), insomnia (6.4%), skin sensitivity (5.6%), and low libido (1.5%). Heart rate is survivorship-biased; skin sensitivity is genuinely dose-dependent.

Tirzepatide has the best overall benefit profile — leading on energy, inflammation reduction, blood pressure, mental clarity, and joint pain. It’s also the most satisfying switch destination (36% positive sentiment).

Women report hair loss at 2–3× the overall rate across all three drugs. Confidence improvement (11.4% of female posts) is the largest gender-specific benefit gap. PCOS improvement (n=290) is entirely absent from clinical trial data.

Reddit predicted a Phase 3 finding before publication. Our data showed retatrutide skin sensitivity at 5.6%, consistent with Phase 2 (7%). TRIUMPH-4 later confirmed 20.9% at 12mg — validating the dose-dependent pattern visible in this dataset.
